Oklahoma Vital Records maintains the records for 出生s 和 死亡s that occurred in the state of Oklahoma.

Vital Records will offer a Will Call service, giving individuals the option to pick up orders placed 在线 for 出生 和 死亡 records in-person. Depending on your record request, eligibility is required to perform a search for the record. Failure to include all required items for eligibility or submitting incomplete applications will cause delays in completing your request. Please see below to prepare for your visit:

费用

The fee for the search of a 出生 or 死亡 record is $15. This fee will pay for the issuance of one certified copy of the certificate to the qualified applicant if the certificate is located. Additional copies will be issued at a cost of $15 per copy. The searching fee is not refundable or transferable. Please make your check or money order payable to OSDH. Heirloom 出生 certificates are available for a fee of $35. Corrections or additions to vital records require additional fees 和 required documentation. 11月有效. 1, 2016, all non-sealed records of 出生s that occurred more than 125 years from the current date will be open record, as will all records of 死亡s occurring more than 50 years prior to the current date. No proof of eligibility will be required for those records.  An application, photo ID 和 fees will still be required.

OK2Explore

OK2Explore is a free searchable public index of 出生s 和 死亡s that occurred in the state of Oklahoma. It includes information from records of 出生s occurring in Oklahoma 20 or more years ago 和 records of 死亡s occurring in Oklahoma 5 or more years ago. You can search by type of event (出生 or 死亡), date, county, name 和 sex. The database includes information to help you identify whether a record is on file without paying a search fee. The index will indicate whether or not a record exists, but you cannot view the actual 出生 or 死亡 certificate 在线. If you would like to order a copy of a record, the eligibility rules will apply for non-open records.
